Whether it’s a private residence, senior apartment, or Assisted Living & Independent Living community, our team is committed to meeting patients wherever they may be, reducing the need for outside transport and maintaining patient comfort.  

SEAMLESS COLLABORATION & COMMUNICATION: We prioritize collaboration with nurses, caregivers, and healthcare agencies, working together to ensure patients receive the support they need, when they need it. Through regular on-site visits, we maintain open lines of communication, closing the loop of care and fostering a supportive environment for patients and their caregivers. New treatment orders are provided to each agency and case manager on the same day the patient is seen, and provider progress notes communicated electronically with other interdisciplinary members of the care team.  

OFFERING A HOLISTIC APPROACH TO HEALING: Our approach extends beyond wound care alone; we treat the entire patient. In addition to prescribing medication and ordering necessary supplies and equipment, we address underlying factors contributing to impaired wound healing, such as diet and nutrition, infection, and circulation issues. Our providers order necessary diagnostic tests such as wound cultures and ultrasounds, many of which can be performed on-site in lieu of an outpatient visit. These interventions promote holistic well-being, enhance quality of life, and bring evidence-based practice guidelines directly to patients.
