Led with weekly wound rounds, all types of wounds are evaluated and managed by our team of doctors and nurse practitioners. At Healix, work collaboratively with other providers and members of the disciplinary team to treat all wounds & identify at-risk patients before new wounds develop – including the assessment of new admissions during wound rounds. That same day, our wound assessments are then seamlessly integrated into our cloud-based, online portal and with each facility’s weekly wound assessments – eliminating a task commonly delegated to administrative clinicians and an Excel sheet that can take hours of staff time and create errors and omissions.
WEEKLY ON-SITE WOUND ROUNDS: Our Providers conduct on-site weekly wound rounds, replacing the need for outside appointments to a Wound Center. We provide personalized care plans tailored to each patient’s unique needs while meeting compliance standards for every unique care setting. Core services include bedside procedures such as debridement, infection prevention and monitoring, and the utilization of in-house formularies and Part B ordering for wound care products.
A COLLABORATIVE APPROACH WITH ON-SITE WOUND CARE PROVIDERS: Recognizing the importance of collaboration, we also work as consultants with other wound care provider groups servicing long-term care facilities. By sharing information and insights, we ensure continuity of care and adherence to best practice guidelines to optimize outcomes for patients, providers, and facilities.
A COMPREHENSIVE & REMOTE PRESSURE INJURY PREVENTION SOLUTION: Our proprietary program utilizes SEM scanning for objective identification of patients at risk of developing PIs, aligning with the National Pressure Injury Advisory Panel’s (NPIAP) Standardized Pressure Injury Prevention Protocol 2.0 (SPIPP 2.0). Patients determined to be at-risk of PI development are then communicated with facility nurses for the implementation of a clinical decision tree with bundled order sets built into the electronic medical record (EMR). Our customized clinical decision tree serves as a practical guide for facility nurses and administration and aides in the delivery of consistent, evidence-based pressure reduction interventions and subsequent assessments.
BUNDLED ORDER SETS FOR EVIDENCE-BASED PRESSURE REDUCTION STRATEGIES: Fully integrated into Point Click Care (PCC), our bundled order sets provide tiered levels of evidence-based pressure reduction interventions built directly into any EMR. These sets include a comprehensive selection of facility-friendly interventions aimed at preventing PIs, such as specialty mattresses, repositioning schedules, and preventative dressings that adhere to quality metric standard for the Centers of Medicare & Medicaid (CMS). By streamlining nursing assessments and order entry, our bundled order sets empower facility staff to deliver high-quality care with confidence.
OFFERING A COMPREHENSIVE & COST-EFFECTIVE WOUND CARE FORMULARY: To further enhance our approach, we provide in-house wound care formulary products hand-selected by Dr. Rutherford and designed to deliver evidence-based, consistent, and cost-effective wound care products via our distribution partner. These cost-effective products are conveniently ordered via an online portal and monitored remotely by our clinical staff, ensuring the appropriate selection of products with education and providing consistency to nursing staff.
